The Cognitive Decline Partnership Centre will be holding its Annual Meeting on the Tuesday afternoon of the Australian Dementia Forum. Partnership Centre members will be provided with a discount code to apply if attending the Australian Dementia Forum in recognition that a proportion of Forum costs will be met through the Partnership Centre.

Payment of registration fees must accompany all registrations. No registration will be confirmed until payment is received. Payment can be made by the following methods:
If you are making a credit card payment from a country outside Australia, for security reasons you must first advise your banking institution of the impending charge and authorise them to allow the charge to Australia to go through. Please note that charges to your credit card will appear as being from "ICMS Australasia Pty Ltd" on your bank statement.

Cancellations must be made in writing to the forum secretariat. Cancellations received prior to 11 September 2017 will receive a full refund minus a $150 administration fee. Cancellations received after 11 September 2017 will not be refunded. Substitute delegates will be accepted.
Failure to obtain the appropriate visa to travel to Australia does not constitute a refund. The Forum Secretariat must be advised in writing of any alterations or transfers.
While we will endeavour to present the program as advertised, the Forum organisers reserve the right to alter, without prior notice, any of the arrangements, timetables, plans or other items relating to the Forum, for any reasons outside of their reasonable control. The organising committee will not be liable for any loss or inconvenience caused as a result of such alterations. In the event of unforeseen circumstances, the organising committee will not accept responsibility for the loss of monies caused by delays. Participants are advised to take out personal travel insurance and to extend their policies to cover personal possessions. Individuals are not covered by the organising committee against cancellations of bookings or the theft or damage of belongings.
